The Cook County Clerk's Office has received your application for a mail ballot. We are unable to process it at this time for the following reason(s) marked below.

MAIL BALLOT APPLICATION ISSUE(S):
-Your did not sign your application
-Your signature does not match your voter registration card
-You did not complete the application
-Your application does not have an original signature. We are unable to accept copies.
-You must each sign a separate mail ballot application.
-We do not accept Power of Attorney signature. Voter must make their mark or sign personally.
-You mailed in the wrong application.
-Your application was received after the deadline.
*Application must be received by October 29, 2020.

VOTER REGISTRATION ISSUE:
-You are not registered to vote in suburban Cooke County; or you are registered, but not ad the address given on your application.
*You can also register to vote online at https://ova.elections.il.gov/, using your Illinois driver's license or State ID; the deadline to do so is October 18, 2020 for November 3 2020 GENERAL ELECTION.
*After October 18, 2020 your options are to go to a Grace Period voting location or to your polling place on Election Day where you can register and vote at the same time. To register at a Grace Period site or at your polling place on Election Day, you will need to show two piece of identification (a Photo ID is not required).
